It forms part of his whole approach to analysis. In ‘The Aims of Psychoanalytical Treatment’ (1962a: 169) he states ‘The essential thing is that I do base my work on diagnosis’. He distinguishes between ‘standard analysis’ and ‘working as a psychoanalyst’, the crucial issue being whether the patient has sufficient ego integration based on a good-enough beginning. In a talk shortly before he died Winnicott said ‘Psychoanalysis is not just a matter of interpreting the repressed unconscious; it is rather the provision of a professional setting for trust, in which such work may take place’ (1986: 115).
Winnicott did not write extensively about the psychoanalysis of neurotic patients, suggesting not only that this was not his primary concern, but that he agreed with the prevailing view of treatment of ‘whole persons’ who could relate to others and recognize their inner reality and its conflicts. His interest was in those conditions, depression, the antisocial tendency, psychotic and borderline phenomena whose origins lay before whole person functioning. His interest in depression and the antisocial tendency for instance led him to investigate situations where a good-enough beginning was then lost.
Over time this becomes a structure in the baby’s mind. The Bionian baby has some sense of an ‘other’ into whom projections may be put, but for Winnicott processes of projection and introjection and the development of the drives follow the establishment of the early infant–mother setup. Projections at the beginning derive from the inner world of the mother, a process which can be seen as precarious and risky. Ogden gives an account of these differences between Winnicott and Bion as observations about human functioning, within a perspective of the relational environment.
